Hidalgo Psychiatry is required by law to maintain the privacy of your Protected Health Information (PHI). PHI includes information that identifies you and relates to your past, present, or future physical or mental health condition, the provision of health care to you, or payment for health care. Uses and disclosures for marketing purposes.
Disclosures that constitute a sale of PHI.

| Right to Confidential Communications | You may request that we communicate with you about medical matters in a certain way or at a certain location (e.g., calling only your cell phone or sending mail to a specific address). |
| Right to an Accounting of Disclosures | You may request a list of certain disclosures of your PHI that we have made. |
| Right to a Paper Copy | You have the right to obtain a paper copy of this Notice upon request. |
Hidalgo Psychiatry is required to abide by the terms of the Notice currently in effect. We reserve the right to change the terms of this Notice at any time. If a material change is made, the new Notice will be available upon request and posted prominently on our website.
If you believe your privacy rights have been violated, you may file a complaint with our Privacy Officer or with the U.S. Department of Health and Human Services (HHS). We will not retaliate against you for filing a complaint.

Phone Number: The mobile number you provide to receive SMS messages.
Message Interaction Data: Delivery and engagement metrics (e.g., delivery status, response keywords).
Consent Records: Timestamps and methods of opt-in (web form, keyword, verbal, paper or e-mail).

Send SMS messages (Scheduling, Customer Care, Event Updates, and Public Service Announcements) per your opt-in preferences.

One-to-One Consent Requirement Under the January 2024 FCC ruling, opt-in consent applies solely to Hidalgo Psychiatry. We do not share your consent with third parties or affiliates unless you expressly agree.
Message Frequency & Data Rates The SMS message frequency will vary but will not be more than 3-5 messages per day unless there is a notification event. Message and data rates may apply; check your carrier’s terms.
